Dental Availment Procedure
For Intellicare, Maxicare, Valucare, Medicard, Cocolife & Sun Life Grepa patient.
Dental Availment:
Requirements:
· HMO Card
· Valid Government ID or Company ID
Step 1:
Begin by verifying if the Dental Clinic is accredited with your HMO card.
Step 2:
Check the list of accredited dentist through the provided link. If uncertain about the type of doctor needed, proceed to the hospital's HMO section or proceed with the HMO Coordinator in the hospital.

Step 3:
Schedule your dentist appointment with your chosen accredited dentist.
Note: Dentists rarely accept walk-in clients due to their limited capacity; it's better to have it scheduled.
Step 4:
Advanced LOA is not applicable to dental availment, Dental clinics will secure LOA approval after the initial consultation. If the dental clinic encounters difficulty in securing an LOA after the initial consultation, please notify us immediately.
*Personnel Protective Equipment (PPE) is not covered by our HMO coverage, we can choose to look for other dental clinic who is not asking for PPE fee.
